Ordering a Playchute? This is the information we need:

1. Size of playchute
2. Colours, in the order in which you want them sewn
3. Centre cap?
4. Any letters or logos needed (see Logos, Letters, Numbers and Banners)
5. Any extra booklets or accessories (see Booklets and Accessories)
6. Your telephone number and/or e-mail address
7. Delivery and invoice addresses
